This form must be filled out so accommodations can be made. If you have any questions or need assistance filling out the form, please reach out to the nurse at your child's building.

Snack Program
Holden R-3 is proud to participate in the Child and Adult Care Food Program (CACFP), which allows us to provide a free "super snack" to all student each day, immediately after the school day. Each meal includes milk, fruit, vegetable, protein and grain - giving students a healthy boost before heading home or to after-school activities.
